I agree with Boshuda, the "stash" is great. My ref does a good job of sticking in the whistle when in dialog, and I can just see, in my mind, the stash twitching as the guy talks.
The only thing I *might* suggest, and don't change anything if you don't wanna, is that one side keep the current coloration, the other be something like red or very yellow - this NPC appears in a club with different colored lights and that might convey that feel a little more.

It looks nice, brother! I'd like to see you experiment with getting rid of the "ink" lines though... I don't think this piece needs them.
